SYARAT PERMOHONAN KTP :


PEMBUATAN KTP BARU :

  1. KK / Fotocopy KK terbaru
  2. Akta Kelahiran / Fotocopy Akta kelahiran

*catatan : pembuatan KTP baru untuk warga yang belum pernah mempunyai KTP

PEMBUATAN KTP BARU JIKA RUSAK / PENGGANTIAN ATAU HILANG :

  1. KK / Fotocopy KK terbaru
  2. KTP fisik lama (apabila rusak)
  3. Surat Kehilangan dari Kepolisian (apabila hilang)

 

SYARAT PERMOHONAN KK :


PEMBUATAN KK BARU (apabila data salah/Tidak sesuai) :

  1. KTP asli dan Fotocopy
  2. Bukti Pendukung Terbaru (seperti : Fotocopy Ijazah terakhir, Fotocopy Surat Nikah )


PEMBUATAN KTP BARU JIKA RUSAK / HILANG :

  1. KTP asli dan Fotocopy
  2. Bukti Pendukung Terbaru (seperti : Fotocopy Ijazah terakhir, Fotocopy Surat Nikah )
  3. Fotocopy Surat Kehilangan dari Kepolisian
